How to Submit a Request:

  1. Complete our Records Request Form.
  2. Include detailed information about the records you are seeking, such as dates, specific documents, or topics to help us locate the information promptly.
  3. Submit your request online, mail, or in person at our District Office during business hours.

Response Time:
Upon receiving your request, we will acknowledge it within five business days. Our staff will review your request and notify you of any costs associated with fulfilling it, as well as provide an estimated timeline.

Fees:
There may be fees for copying, scanning, or delivering documents, in accordance with state law.
